DescriptionPsychotherapy is a complicated, dynamic neurobiological interaction or “dyadic dance” that takes place between two people. Unconscious physical and emotional memories from our early attachment experiences shape how we “dance,” determining patterns of eye contact, proximity or distance, autonomic arousal, internal sense of safety or danger, trust and distrust. When our clients’ early experiences involve neglect and abuse, they are motivated by competing desires and relational anxieties. While attempting to trust or endure disappointment and misalignment in treatment, strong emotional memories of danger, shame, hurt, and betrayal infiltrate the therapeutic partnership. Rather of improving the situation, the therapist’s displays of empathy or attempts to process what transpired during the transference can appear to worsen it.
Interpersonal neurobiology provides therapists with a fresh perspective into how we might shift the post-traumatic “dance” without having to intellectually understand empathic failures and misattunements. Keeping a smooth dancing beat with our clients necessitates understanding how to monitor and manage our own nervous systems and physical experiences, as well as theirs. We may learn to create moments of attunement and connection even with our most difficult clients by applying findings from the study of interpersonal neurobiology and Sensorimotor Psychotherapy, a body-centered talking treatment for attachment and trauma.

Janina Fisher, Ph.D., is a certified clinical psychologist and former lecturer at The Trauma Center, which Bessel van der Kolk developed as a research and therapy facility.
Dr. Fisher is a trauma specialist who has been treating individuals, couples, and families since 1980.
She is a past president of the New England Society for the Treatment of Trauma and Dissociation, an EMDR International Association Credit Provider, the Sensorimotor Psychotherapy Institute’s Assistant Educational Director, and a former Harvard Medical School Instructor.
Dr. Fisher talks and educates on themes relevant to the integration of neurobiological research and newer trauma treatment paradigms into established therapy methods on a national and worldwide scale.
She is the co-author, with Pat Ogden, of Sensorimotor Psychotherapy: Interventions for Attachment and Trauma (2015), as well as the author of Trauma Survivors’ Fragmented Selves: Overcoming Internal Self-Alienation (2017) and the forthcoming Working with the Neurobiological Legacy of Trauma (in press).
