This presentation will go over the examination and treatment of the anticoagulated patient when operations that require interruption of therapy are scheduled.

Explain “bridge” therapy with injectable anticoagulation after warfarin discontinuation, as well as unique concerns for renal patients.
Explain how to handle procedures with newer direct oral anticoagulants.
Michelle Johnson has been a pharmacist at M Health Fairview since 2014. She is the Ambulatory Anticoagulation Clinics’ Pharmacist Supervisor. Michelle earned her Doctor of Pharmacy degree in 1997 from the University of Minnesota College of Pharmacy. Michelle earned her BPS Ambulatory Care certification in 2014. Michelle formerly worked for the Fond du Lac Reservation in Cloquet, MN, for two and a half years, where she helped establish and implement collaborative protocols and launched a pharmacist-led anticoagulation program. Michelle worked at Goodrich Pharmacy for 14 years. While at Goodrich, she developed and implemented collaborative procedures, as well as established and implemented MTM at two new clinical practice sites inside a partnered clinic. Public health, vaccines, and women’s health are further topics of focus. For many years, she has worked with the Minnesota Department of Health on their Mark of Excellence Program, MN Vaccines For Children, and incorporating pharmacy into disaster preparation planning. Michelle has been a board member of the Minnesota Pharmacist Association since 2014. She also serves on the Minnesota Department of Health’s Pandemic Preparedness Workgroup and the National Association of County and City Health Officials Pharmacy Partners Workgroup. She is also a member of the American Pharmacist Association, and her work in JAPHA was published in 2010. She was named a Distinguished Young Pharmacist of the Year by MPHA in 2002, a One to One Patient Counseling Recognition Recipient by APHA in 2007, and an Individual Immunization Champion Award winner by APHA in 2011.
